Meet Bill and Ginny. He is 97, she is 94. And they just celebrated their 75th wedding anniversary! Ginny still tap dances 3 days a week: one day spent taking dance lessons and the other two as a volunteer at local skilled nursing facilities where she tap dances for senior citizen to (in her own words) “cheer the old people up”. Bill, who is an ex-chair of a physics department uses his free time to volunteer at local schools to tutor kids in math and science. Bill and Ginny, who gave me permission to share their incredible story, came to my medical dermatology clinic today without any aid of a walker or a cane. In fact, Bill still exercises on his “stationary bike” for at least 30 minutes each day. .

In medicine we receive the gift everyday to witness such incredible stories and to learn from them. While typically I post my own cases here, I am making an exception today to show a patient of my husband’s. I have posted a #vitiligo#beforeandafter previously, but this one is definitely more dramatic! He showed this image during his talk about JAK inhibitors in #dermatology at this meeting, and I realized it was one that I just had to share.
This woman has had near complete repigmentation with off-label tofacitinb plus phototherapy (prescribed UV light). And yes, this is the same medicine that has helped many of my patients with #alopeciaareata grow hair.
Vitiligo is an autoimmune condition in which melanocytes (pigment-making cells) get destroyed. As it turns out, the signaling pathways that are responsible for loss of pigment in vitiligo are similar to those responsible for loss of hair in alopecia areata. This is the reason that the same medicine can be effective for two different disorders.
In the coming years we will see more and more about these medicines. It’s an exciting time for a group of disorders that have traditionally not had many options for treatment.

could tell you how long i went w/out seeing my best friend bc i felt too ashamed to be seen. could recall fiercely avoiding mirrors, or list all the horrible words i had for myself, or tell you about not being able to look my gf in the eye when she flew in for a surprise visit- or how i dread to think how long i’d have been prepared to go w/out seeing her had she not done this.
the acne shame monster is v real and v big, and around this time too did a well-meaning and important person in my life tell me to... essentially get over it and get on with it. said person has never so much as had a pimple, let alone severe cystic acne, so i’m forgiving of the fact it might be a little incomprehensible to them just how drastically it can change your inner and outer world. For a while it had me stripped bare of anything resembling self esteem or self worth and all i wanted was to hide away. My gf says where i’m at now and where i was back then are polar opposites and she couldn’t be more right and i couldn’t be more grateful to not be back there- but i didn’t get to here on my own. There are a handful of people who’s love and support and remarkable ability to say just the right things at just the right times set me off on the path to being ok. i think about those conversations and acts of kindness now and am amazed by how far and deep they must have traveled. and there are a hundred things i’d go back and tell myself if i could, which is why it’s so damn meaningful to be able to tell any of you who may need to hear it now.
at the top of that list would be an absolute guarantee that the people who matter don’t care about your skin beyond caring about your health and happiness. that drawing the short straw in skin genetics does not make you unlovable.

The procedure shown in this video is Sclerotherapy.
Sclerotherapy is a minimally invasive procedure that treats varicose veins and spider veins.
Sclerotherapy involves injecting a solution (sclerosant) directly into the vein. The sclerotherapy solution causes the vein to scar, forcing blood to reroute through healthier veins. The collapsed vein is reabsorbed into local tissue and eventually fades.
After sclerotherapy, treated veins tend to fade within a few weeks, although occasionally it may take a month or more to see the full results. In some instances, several sclerotherapy treatments may be needed.

Everyone knows training as a doctor is grueling. What people can’t even begin to fathom is the sheer volume of noxious 🐂💩we have to deal with and clean up on a daily basis, and the extent to which we get 💩on by pretty much everyone. And sometimes the 💩⛈ hits like a monsoon, and you’re just glad you got out of the hospital before it’s time to go to bed 😪. By the end of this year, I’m going to be an expert at managing social disasters and logistical nightmares 🧙🏻♂️. That’s why we get paid the “big 🦌’s” after residency 🥴🤣...oh yea and that’s also why we become jaded and cynical 😑. Luckily, all will be fixed after residency is finished...hopefully 😬

Internal medicine vs. family medicine: what’s the difference?
When I worked in family medicine I treated patients of all ages - babies and up. Lots of sick kids with ear infections, well child and sports physicals, plus adult sick visits and adult physicals. I also did chronic care management which included treating & monitoring hypertension, type 2 diabetes, high cholesterol, and much more. •••
Internal medicine and family medicine have a lot of overlap, but I’m only treating adults, and this mostly includes managing chronic diseases as mentioned above. Many IM physicians/PAs/NPs work in hospitals only or both hospital & outpatient clinic. I work outpatient only (although a part of me did want to work in a hospital setting while I was in PA school!) •••
Beginning next month I’ll be back to dermatology full time! I’m so excited to start at a new, innovative practice! I’m happy that I was able to supplement a rough patch at my other job with internal medicine, something that I don’t think I would have had the confidence to do without my previous experience in family medicine. “He makes all things work together for my good” Romans 8:28 ⚡️ Scrubs: @jaanuubydrneela
